Note on the Application

Please create ONE single PDF containing all application documents. The order should be:

  • Application Form
  • Informed Consent
  • Questionnaire(s)
  • Other documents (if applicable)

If you yourself want to check on the ethical aspects of a research proposal you can do so by completing the checklist included in the application form.

Please send the completed documents, including all the required attachments, by to the Office of the Ethics Commission.

The Zivilklausel checklist included in the Zivilklausel application form can also be used simply to inform the Ethics Commission about a research proposal or to check on a research proposal in the light of the Zivilklausel yourself.
